Nurse 2 Nurse Staffing is committed to providing a Drug-Free work environment. All applicants being considered for employment will be required, as a condition of employment, to take and pass a drug screening test. By selecting “Yes” to “Have you read, understood, and do you agree to comply with our policies (located at nurse2nursestaffing.com/application-consent)?”, I agree to take such tests as may be required in the future and to hold Nurse 2 Nurse Staffing harmless of the results.
Release of Medical Information
By selecting “Yes” to “Have you read, understood, and do you agree to comply with our policies (located at nurse2nursestaffing.com/application-consent)?”, I authorize the release of ANY and ALL medical information pertaining to my employment to Nurse 2 Nurse Staffing AND to its client facilities pursuant to this application for employment.
Permission to contact previous employers and schools
By selecting “Yes” to “Have you read, understood, and do you agree to comply with our policies (located at nurse2nursestaffing.com/application-consent)?”, I hereby authorize any person, company, and or educational institution I have listed as a reference on my employment application to fully disclose in good faith any information they may have regarding my qualifications for employment. I will not hold any prospective or former employer, educational institutions, nor any other persons giving references, liable for the exchange of this information along with any other reasonable and necessary information that is necessary and inherent to the employment process.
Permission for Background Check
By selecting “Yes” to “Have you read, understood, and do you agree to comply with our policies (located at nurse2nursestaffing.com/application-consent)?”, I hereby authorize Nurse 2 Nurse Staffing, and their designated agents and representatives to conduct a review of my background causing a consumer report and/or an investigative consumer report to be generated for employment purposes, retention purposes, promotion, or reassignment unless revoked in writing. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to, the following areas: Verification of social security number; current and previous residences; employment history including all personnel files; education including transcripts; character references; credit history and reports; criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions; birth records; motor vehicle records to include traffic citations and registration; workers compensation for employment; and any other public records. I authorize Nurse 2 Nurse Staffing to conduct interviews with third parties relative to my character, general reputation, personal characteristics or mode of living. I further authorize any individual, company, firm, corporation, or public agency (including the Social Security Administration, military, and law enforcement agencies) to divulge any and all information, verbal or written, pertaining to me to the background service or its agents. I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources. I hereby release the background service, the Social Security Administration, and its agents, officials, representatives, or assigned agencies, including officers, employees, or related personnel both individually and collectively, from liability to the extent permitted by law for damages of whatever kind, which may, at any time, result to me, my heirs, family, or associates because of compliance with this authorization and request to release. If an investigative consumer report is conducted I understand that I have the right to request additional information about the
nature of the report and a copy of the report by calling the background service.
POLICIES ACKNOWLEDGMENT & COMPLIANCE
By selecting “Yes” to “Have you read, understood, and do you agree to comply with our policies (located at nurse2nursestaffing.com/application-consent)?” on my application, I am agreeing that I have read, understand, and will comply with the Drug-Free Environment, Medical Authorization Information, and Permission to Contact Previous Employers policies and release the requested information as noted above to Nurse 2 Nurse Staffing directly related to my application for employment. I attest that I am the applicant and the information provided in this application is complete, true, and accurate, to the best of my knowledge, and I understand that any misrepresentation, falsification or willful omission shall be sufficient reason for refusal of, or dismissal from employment and or employment consideration, and may be a violation of state law(s) that could result in civil penalties. I further understand that this employment application is not a contract for employment and that any individual who is hired may voluntarily leave employment At-Will and that employment by Nurse 2 Nurse Staffing may be terminated or withdrawn at any time with or without notice or cause. I understand that any oral or written statements to the contrary are hereby expressly disavowed and should not be relied upon by the prospective or existing employee. I understand and acknowledge that I may be required to submit to a physical examination as a condition of employment with Nurse 2 Nurse Staffing, should an offer of employment be extended to me.
